Abstract

     บทความนี้มีวัตถุประสงค์ 3 ประการ ประการแรก ได้แก่ เพื่อนำเสนอผลสรุปของการวิจัย เรื่อง แบบแผนความเชื่อด้านสุขภาพของ Marshall H. Becker ในการวิเคราะห์พฤติกรรมสุขภาพของผู้ป่วยโรคความดันโลหิตสูงที่อาศัยอยู่ในเขตชุมชนเมือง จังหวัดพิษณุโลก ประการที่สอง เพื่อนำเสนอผลการวิเคราะห์ระบาดวิทยาทางสังคมของผู้ป่วยโรคความดันโลหิตสูง และประการสุดท้าย เพื่อเสนอแนวทางควบคุมโรคความดันโลหิตสูง ในกลุ่มผู้ป่วยเป้าหมาย ผู้เขียนต้องการชี้ให้เห็นถึงความสำคัญของการจัดการกับปัญหาโรคความดันโลหิตสูงของผู้ป่วยในเขตเมืองแบบองค์รวม โดยคำนึงถึงปัจจัยทางวัฒนธรรม บริบททางสังคม และการนำเครื่องมือทางสังคมศาสตร์ใหม่ๆ อาทิ แผนที่ระบาดวิทยาทางสังคมมาใช้เพื่อจัดการกับความเสี่ยงทางวัฒนธรรมสุขภาพ โดยใช้ระเบียบวิธีวิจัยผสมผสาน ประกอบด้วย (1) การศึกษาเชิงปริมาณ โดยใช้กลุ่มตัวอย่าง คือ ผู้ป่วยโรคความดันโลหิตสูงในชุมชนพันปี จังหวัดพิษณุโลก ซึ่งเป็นชุมชนเขตเมืองที่พบปัญหาผู้ป่วยโรคความดันโลหิตสูงสะสมน้อยที่สุด เก็บข้อมูลโดยใช้แบบสอบถามความเชื่อด้านสุขภาพแบบปลายปิด กับผู้ป่วยที่คัดเลือกตัวอย่าง ตามตารางของทาโร ยามาเน จำนวน 56 ราย และวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ การแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน (2) การศึกษาเชิงคุณภาพ ประกอบด้วย การใช้แนวการสัมภาษณ์เชิงลึกกับกลุ่มเป้าหมายจำนวน 10 ราย ที่คัดเลือกจากผู้ที่มีความดันโลหิตสูงในระดับที่ควบคุมได้ และใช้การวิเคราะห์เนื้อหา เขียนบรรยายแบบเล่าเรื่อง และนำเสนอเป็นรายประเด็นของการศึกษา และ (3) การวิเคราะห์ระบาดวิทยาทางสังคม โดยบูรณาการผลการวิเคราะห์ข้อมูลเชิงปริมาณและเชิงคุณภาพ ผ่านตัวแปรอายุและภาวะแทรกซ้อนของโรค ก่อนสังเคราะห์ข้อมูลและนำเสนอในรูปของแผนที่ทางสังคม เพื่อแสดงระบาดวิทยาทางสังคมของผู้ป่วยความดันโลหิตสูง
     ผลการศึกษา พบว่า (1) ปัจจัยด้านการรับรู้ความรุนแรงของโรค มีค่าเฉลี่ย 4.48 (S.D. = .327) มากที่สุด (2) ด้านระบาดวิทยาทางสังคม โดยการจำแนกตามช่วงอายุ และภาวะแทรกซ้อน พบการกระจุกตัวของผู้ป่วยในสองพื้นที่ คือ ชุมชน “น้ำค้าง” และ “สีนวล” (3) แนวทางการควบคุมโรคความดันโลหิตสูงในเขตเมือง ควรพิจารณาระบาดวิทยาทางสังคม ร่วมกับการวิเคราะห์บริบททางสังคม ปัจจัยด้านสังคม วัฒนธรรม อาทิ ความหลากหลายของอาหารที่มีในท้องถิ่น และบทเรียนที่ถอดจากประสบการณ์ของผู้ป่วย ซึ่งประกอบด้วยการควบคุม จัดการและพฤติกรรมการดูแลสุขภาพตนเองตามบริบททางสังคมวัฒนธรรม ได้แก่ การบริโภคอาหาร การออกกำลังกาย การควบคุมอารมณ์และการจัดการกับความเครียด

     This article had 3 objectives. First, it aimed to present the conclusive findings of the research named “the study of hypertension patients according to Marshall H. Becker’s health belief model in urban communities in Phitsanulok”. Second, it aimed to present social epidemiology among hypertension patients. Finally, it aimed to propose practical guidelines for controlling high blood pressure in target population. The authors highlighted the crucial of using holistic approach concerning cultural risk factors, social context and the application of new social research tools called “Social Epidemiology Social Geographical Mapping (SESGM) for the study of situation and health behaviors of the target patients. Mixed method included: (1) quantitative research was applied to collect data from 56 samples who had been identified by using Taro Yamane sampling table. First, the authors adopted Becker's Health Belief Model (HBM) for developing research tools. The descriptive statistical analysis was applied. The quantitative findings were presented in forms of frequencies, percentage, median and standard deviation. (2) Then, qualitative research was carried out to collect data from 10 middle level and controlling blood pressure cases. These cases were individually interviewed by using interview guideline. The data was analyzed by using content analysis and narrative technique. (3) Finally, SESGM was synthesized based on social factors including age and complication symptoms.
     Findings included: 1) Most patients perceived severity at an average of 4.48 (S.D. =.428), 2) The social epidemiology revealed the highest concentration on areas of “Namkhang” and “Si-Nuan”, 3) The authors suggested that a practical guideline for controlling high blood pressure for these hypertension patients should be based on the consideration of socio-cultural context, self-care and social epidemiology. These included consumption behaviors such as food controlling based on varieties of food available in patients’ habitat, self-care behaviors such as exercise, emotion and stress management. They suggested that the good practical guidelines for hypertension patients in urban community should be based on good practice developing from experiences of hypertension patients, health behaviors and socio-cultural context.
