Abstract

     การขยายความครอบคลุมของวัคซีนเป็นตัวชี้วัดที่สำคัญ สะท้อนให้เห็นถึงการดำเนินงานควบคุมและป้องกันโรค และการสร้างเสริมภูมิคุ้มกันโรค กระทรวงสาธารณสุขกำหนดมาตรฐานความครอบคลุมไว้ที่ร้อยละ 90 แต่ในอำเภอหล่มเก่า จังหวัดเพชรบูรณ์ มีอัตราความครอบคลุมของวัคซีนขั้นพื้นฐานของชาวม้งต่ำกว่าร้อยละ 75 ในทุกชนิด การที่ผู้ปกครองชาวม้งไม่พาเด็กมารับวัคซีนหรือมาไม่ตรงตามนัดนั้นเป็นผลมาจากหลายปัจจัย รวมไปถึงปัจจัยด้านความเชื่อเรื่องการเจ็บป่วยที่ส่งผลต่อการมารับบริการสร้างเสริมภูมิคุ้มกันโรคด้วยวัคซีน โดยที่ผ่านมายังไม่มีการศึกษาที่ชัดเจนเกี่ยวกับความเชื่อดังกล่าว
     การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างความเชื่อเรื่องการเจ็บป่วยของผู้ปกครองชาวม้งและการมารับบริการสร้างเสริมภูมิคุ้มกันโรคด้วยวัคซีนขั้นพื้นฐานในเด็กอายุต่ำกว่า 5 ปี รูปแบบการศึกษาแบบผสมผสาน ประชากรที่ใช้ในการศึกษาครั้งนี้คือ ผู้ปกครองชาวม้งที่มีเด็กอายุต่ำกว่า 5 ปี จำนวน 265 คน การศึกษาวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยา: กลุ่มตัวอย่าง คือ ผู้ให้ข้อมูลสำคัญ จำนวน 18 คน เก็บรวบรวมโดยแบบสัมภาษณ์เชิงโครงสร้างด้วยการสัมภาษณ์แบบเจาะลึก วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหาและการศึกษาวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง: กลุ่มตัวอย่าง คือ ผู้ปกครองชาวม้งที่มีเด็กอายุต่ำกว่า 5 ปี จำนวน 115 คน ใช้แบบสัมภาษณ์ เก็บรวบรวมข้อมูลโดยผู้วิจัยเป็นผู้สัมภาษณ์ด้วยตนเอง และใช้แบบคัดลอกข้อมูล เพื่อบันทึกข้อมูลประวัติการมารับวัคซีน และประวัติการตั้งครรภ์จากเวชระเบียน วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์การถดถอยลอจีสติคเชิงพหุ ดำเนินการระหว่างเดือนธันวาคม พ.ศ. 2559 – เดือนเมษายน พ.ศ. 2560
      ผลการศึกษาพบว่าลักษณะความเชื่อเรื่องการเจ็บป่วยของผู้ปกครองชาวม้งสามารถแบ่งออกเป็น 2 กลุ่ม คือ ความเจ็บป่วยที่มีสาเหตุการเกิดโรคมาจากธรรมชาติ (ความไม่สมดุลของ ร่างกาย สิ่งแวดล้อม และสิ่งที่ทำให้เกิดโรค) และสาเหตุการเกิดโรคมาจากสิ่งที่นอกเหนือธรรมชาติ ชาวม้งนั้นมีความเชื่อในเรื่องภูตผี และนับถือวิญญาณต่าง ๆ โดยเฉพาะวิญญาณของบรรพบุรุษจะคอยส่งผลดีและผลร้ายกับพวกเขา รวมทั้งการเกิดอาการเจ็บป่วยต่างๆ อัตราการมารับบริการสร้างเสริมภูมิคุ้มกันโรคด้วยวัคซีนขั้นพื้นฐานครบตามช่วงอายุเท่ากับ ร้อยละ 67.8 ผู้ปกครองที่มีความเชื่อเรื่องการเจ็บป่วยว่ามีสาเหตุการเกิดโรคมาจากธรรมชาติมีความสัมพันธ์กับการพาบุตรหลานมารับบริการสร้างเสริมภูมิคุ้มกันโรคด้วยวัคซีนขั้นพื้นฐานในเด็กอายุต่ำกว่า 5 ปีครบตามช่วงอายุอย่างมีนัยสำคัญทางสถิติ (ORajusted=5.9; 95% CI 1.9 ถึง 18.1) และปัจจัยอื่น ๆ ได้แก่ ผู้ปกครองอายุมากกว่าหรือเท่ากับ 20 ปี (ORadjusted =5.5; 95% CI 1.6 ถึง 19.6) การฝากครรภ์ก่อนคลอดมากกว่าหรือเท่ากับ 5 ครั้ง (ORajusted =3.9; 95%CI 1.3 ถึง 11.8) นับถือศาสนาพุทธ (ORajusted=5.3; 95% CI 1.1 ถึง 27.0) ไม่เชื่อว่าการฉีดวัคซีนเข้าสู่ร่างกายถือว่าเป็นการขัดต่อความเชื่อของชาวม้ง (ORajusted =3.8; 95%CI 1.3 ถึง 10.5) การดำเนินงานเพื่อแก้ไขปัญหาดังกล่าว เจ้าหน้าที่ที่เกี่ยวข้องควรให้ความสำคัญกับกลุ่มชาวเขาหรือชาวม้งมากขึ้น และเร่งสร้างความเข้าใจในเรื่องกลไกและกระบวนการของการสร้างภูมิคุ้มกันโรค ซึ่งจะนำไปสู่ความเชื่อเรื่องการเจ็บป่วยที่ถูกต้อง และเพื่อให้อัตราการเข้ารับบริการมากขึ้น นำไปสู่สุขภาพอนามัยที่ดีของเด็ก

      Expanding the coverage of vaccines is a key indicator to show the prevention and control of communicable diseases and the progress of vaccines and immunization. The vaccination coverage of Hmong people is less than 75% in all types in Lom Kao District, Phetchaboon, which is lower than the standard rates of vaccination for children (90%) by the Ministry of Public Health. There are several important factors that influence parent’s making the vaccine decision However, there is no clear association between Hmong parents' illness belief and compliance with immunization schedule for children under 5 years of age.
     The purpose of this study is to find the relationship between Hmong parents' illness belief and compliance with immunization schedule for children under 5 years of age. The study design was mixed methods research design. The population was 265 Hmong parents of children under 5 years of age I) the phenomenology study with 18 persons conducted key informant interviews. Using structured interview in in-depth interviews to provide insight on the Hmong parents' illness belief. Qualitative data were analyzed using content analysis and II) a cross-sectional analytical study with 115 Hmong parents by a face-to-face interview. We also extracted vaccination and antenatal care information from Hosxp PCU primary care unit database. Quantitative data were analyzed using multiple logistic regression to investigate the relationship. During December 2016 - April 2017
     The Hmong parents' illness belief can be divided into two groups by illnesses that are caused by natural history of disease (imbalance of the host, the environment, and the agent), and the caused by something beyond nature. Hmong people have a culture on animistic beliefs and ancestral spirits that spirits are around them. It can create good results and illness on them especially the ancestral spirits. The compliance with immunization status of children rate was 67.8%. The compliance with immunization status of children was statistical significance to associate with Hmong parents' who belief illness caused by nature (ORajusted = 5.9; 95% CI  1.9 to 18.1). Other factors with statistically significantly associated were; parents aged over 20 years (ORajusted = 5.5; 95% CI = 1.6 to 19.6), attending antenatal cares more than 5 times (ORajusted = 3.9; 95% CI = 1.3 to 11.8), Buddhist parents (ORajusted = 5.3; 95% CI = 1.1 to 27.0), and a thought of vaccinations does not violate Hmong beliefs (ORajusted = 3.8; 95% CI = 1.3 to 10.5) To increase the number of children with the compliance with immunization status, public health officers, and network partners should describe the mechanisms and processes of disease to Hmong parents to understand in order to increase service rates and lead to good hygiene of children.
