ชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในทศวรรษหน้า (พ.ศ.2563):ความเห็นจากประชาชนจังหวัดขอนแก่น The Appropriate Dental Core Package: Khon Kaen People’s Opinion (2020)

        ประเทศไทยได้มีการใช้ชุดสิทธิประโยชน์ทางทันตกรรมมาตั้งแต่ปี พ.ศ. 2544 จนถึงปัจจุบัน ซึ่งชุดสิทธิประโยชน์ทางทันตกรรม
ที่เหมาะสม ถือว่าเป็นยุทธศาสตร์หนึ่งของระบบงานทันตสาธารณสุขที่สามารถช่วยปรับปรุงและยกระดับคุณภาพชีวิตของประชาชน
ให้ดำเนินชีวิตได้อย่างปกติสุข ที่ผ่านมายังไม่เคยมีการศึกษาถึงความเหมาะสมของชุดสิทธิประโยชน์ทางทันตกรรมที่ใช้ในปัจจุบัน
ว่าเป็นอย่างไร เป็นแต่เพียงการประเมินสถานการณ์การจัดบริการทางทันตกรรมภายใต้ชุดสิทธิประโยชน์ทางทันตกรรมที่ใช้ในปัจจุบันเท่านั้น ซึ่งศึกษาถึงเฉพาะมุมของการเข้าถึงบริการทันตกรรม และความพึงพอใจของประชาชนต่อการเข้ารับบริการเท่านั้น ไม่ได้มีการวิเคราะห์ถึงความต้องการของประชาชนและผู้มีส่วนได้เสียที่เกี่ยวข้อง เพื่อที่จะได้นำไปพัฒนาชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในอนาคตได้ การศึกษาในครั้งนี้ จึงมีวัตถุประสงค์เพื่อพัฒนาชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในอนาคต (พ.ศ. 2563) ซึ่งเป็นการวิจัยนโยบายสาธารณะ โดยมีการรวบรวมข้อมูลเชิงคุณภาพเป็นส่วนใหญ่ซึ่งใช้พื้นที่จังหวัดขอนแก่นเป็นพื้นที่ในการศึกษาวิจัย แบ่งออกได้เป็น 3 ระยะดังนี้คือ ระยะที่ 1 การพยากรณ์สภาพปัญหาและสถานการณ์ในอนาคต ระยะที่ 2 จัดเวทีรับฟังความคิดเห็นและความเป็นไปได้ ระยะที่ 3 สร้างแบบจำลองสุดท้ายของชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสม โดยนำความเห็นและข้อเสนอแนะของกลุ่มผู้มีส่วนได้เสียทั้ง 3 กลุ่ม ซึ่งประกอบด้วย ผู้ให้บริการ ผู้รับบริการ ผู้กำหนดนโยบาย มาวิเคราะห์และสังเคราะห์ เพื่อจัดทำชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในปี พ.ศ. 2563 ผลการศึกษาแบ่งออกเป็น 3 ระยะเช่นกันโดย ระยะที่ 1 ได้ภาพจำลองชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในอนาคต (พ.ศ.2563) 3 สถานการณ์ ประกอบด้วย 1) ชุดสิทธิประโยชน์ทางทันตกรรมภายใต้สถานการณ์ที่ดีที่สุด 2) ชุดสิทธิประโยชน์ทางทันตกรรมภายใต้สถานการณ์ที่แย่ที่สุด 3) ชุดสิทธิประโยชน์ทางทันตกรรมภายใต้สถานการณ์ที่อยู่กลางๆ ระหว่างดีที่สุดและแย่ที่สุด ระยะที่ 2 นำภาพจำลอง 3 สถานการณ์จาก ระยะที่ 1 นำมาให้ผู้มีส่วนได้เสียให้ความเห็นและปรับแก้ไข ระยะที่ 3 นำภาพจำลองที่ปรับแก้ไขในระยะที่ 2 ให้ผู้มีส่วนได้เสียให้ความเห็นชอบร่วมกันจนได้ชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสมในปีพ.ศ.2563 ซึ่งผู้มีอำนาจตัดสินใจและผู้ที่เกี่ยวข้องสามารถนำไปประยุกต์ใช้กับงาน
ทันตสาธารณสุขได้อย่างเหมาะสมในอนาคต โดยพิจารณาจากสถานการณ์ที่อาจจะเกิดขึ้นที่สอดคล้องกับภาพจำลองชุดสิทธิประโยชน์ทางทันตกรรมที่เหมาะสม ที่ผู้วิจัยได้พัฒนาขึ้นมาได้

        Thailandhas used The Dental Core Package since 2001-2015. The appropriate Dental Core Package was one of the strategic plan for dental public health policy as it could improve quality of life. In the past, there were no studies that showed if the Dental Core Package was suitable or not. The past studies only evaluated situation of dental health services by the Dental Core Package that was used in nowadays. Those studies presented access to dental health services and people’s satisfactions toward dental health services. There were no analysis of stakeholder’s needs for improving the appropriate Dental Core Package in the future. The objective of this study was to develop the appropriate Dental Core Package for the year 2020. This study was public policy research that aggregated qualitative data as the majority. The area of this study is Khon Kaen Province.  There were 3 phases in the study. Phase 1: Predict problem and future situation. Phase 2: Public forum for stakeholder’s opinions and feasibility study. Phase 3: Develop future scenarios of the appropriate Dental Core Package. The study brought together, analyzed and synthesized the opinions of 3 stakeholders (Dental health servicers, People and Policy makers) to create the appropriate Dental Core Package in 2020. From this study, there are 3 phases of results. Phase 1: The results are future scenarios of the appropriate Dental Core Package. 1) Best case scenario. 2) Worst case scenario. 3) Middle of Best case and Worst case scenario. Phase2: Bring future scenarios from phase 1 to stakeholders give opinions for approve them. Phase 3: Bring the result from phase 2 to stakeholders give final consensus and get the appropriate Dental Core Package (2020). The policy makers and others can apply this study to dental public health policy in the future by studying situation analysis that may happen for appropriate Dental Core Package in the research.
